In order to qualify, you must have a documented hearing loss on file and be a Texas resident. The first step is to come by our office and fill out a form for the Department of Assistive and Rehabilitative Services (DARS). If you do not have documented proof of hearing loss, call for an appointment to have your hearing evaluated.
